Job Responsibilities
Work closely with clients, client families, and program managers to guarantee
that clients are receiving the best available care and support, adhering to the
client’s individual personal plan.
Activities of Daily Living include, but are not limited to, feeding, dressing, bathing,
grooming, toileting/incontinence, transfers/repositioning, ambulation, and
exercise.
Housekeeping is required, including laundry, errands/appointments, and
organizing.
Meal planning, preparation, cleanup, and monitor dietary restrictions and
allergies.
Medication administration, scheduling doctor’s appointments, and monitoring
vitals.
Monitoring cognitive status, interacting with, and engaging clients in,
activities/hobbies (both inside and outside the home).
Complete all required documentation in accordance with our agency.
Required to attend mandatory in-service training.
Train and utilize universal precautions as necessary for health and safety.
Conducts well-being checks on clients and reports any changes in client’s
physical or mental status to the program manager immediately.
Staff should be able to kneel and move from sitting, bending, kneeling, or
standing multiple times a day.
Push and pull objects up to 50 pounds.
Climb up and down stairs (depending on the client’s home).
Other Requirements:Relevant Education:
• High School Diploma/ GED
Relevant Experience:
Preferred: 1 year in a comparable human service capacity.
Team-player, reliable, energetic, self-motivated, and well-organized.
